Job Title: Health Information Management (HIM) Clerk

Summary

The Health Information Management (HIM) Clerk is responsible for maintaining the integrity and security of patient medical records, both physical and electronic. This role involves a variety of tasks, including processing incoming medical documents, fulfilling medical record requests, scanning and filing documents, and providing general administrative support to the clinic. The HIM Clerk ensures accurate and timely access to patient information while adhering to HIPAA regulations and clinic policies.

Essential Duties And Responsibilities

  • Medical Record Processing:
    • Retrieve and process incoming faxes from the fax inbox, accurately filing them into the appropriate patient's electronic medical record (EMR).
    • Assign documents to the appropriate clinical staff member for review and action.
    • Gather and scan paper documents from various clinic locations, ensuring accurate and complete capture into the EMR.
    • Maintain a thorough understanding of document organization and filing conventions within the EMR system.

  • Medical Record Requests:
    • Process requests for medical records from outside facilities, ensuring compliance with HIPAA regulations and clinic policies.
    • Prepare and send medical record requests, including completed and signed Release of Information (ROI) forms.
    • Receive and scan incoming medical record requests for our clinic, routing them to the appropriate personnel for processing.

  • Administrative Support:
    • Answer incoming telephone calls to the clinic in a professional and courteous manner.
    • Schedule patient appointments accurately and efficiently.
    • Relay patient messages to clinical staff promptly and accurately.
    • Assist with other administrative tasks as needed.
Qualifications

  • High school diploma or equivalent required.
  • Previous experience in a medical office or healthcare setting preferred.
  • Familiarity with electronic medical records (EMR) systems is highly desirable.
  • Knowledge of HIPAA regulations and patient privacy practices.
  • Strong attention to detail and accuracy.
  • Excellent organizational and time-management skills.
  • Proficient computer skills, including scanning, data entry, and Microsoft Office Suite (Word, Excel, Outlook).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to maintain confidentiality.

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to lift and carry up to 10 pounds.
  • Ability to bend, stoop, and reach.
  • Manual dexterity for operating office equipment.
